Dental Savings Plan Details
All treatments must be paid in full at each visit to keep the plan in effect. Hospital charge is not covered.
All covered services must occur within the year of enrollment and cannot be carried over to the next year.
Savings program is not transferable to another party or uncovered family member and cannot be used in conjunction with any dental or health insurance.
Your effective date is the date you sign up for the plan, and your Membership remains in effect for one year. After the year expiration, you will have the option to renew.
Plan is subject to change yearly and is nonrefundable.
